SMS/Text Messaging Terms

Program: SCCM Care Coordination Messaging. SCCM staff send text messages to enrolled members, caregivers, authorized representatives, and service providers about appointments, home visits, service coordination, and responses to your questions.

Opt-in: You consent to receive texts by agreeing verbally during intake or a call with our staff, by checking the SMS consent box on our intake form, or by texting us first. Consent is not a condition of receiving services.

Frequency: Message frequency varies.

Cost: Message and data rates may apply.

Opt-out: Reply STOP at any time to stop receiving messages. You will receive one confirmation message, and no further texts will be sent unless you opt in again.

Carriers: Carriers are not liable for delayed or undelivered messages.
