I do not have Medicaid, but I do not have a high income or assets and need services. Can I be a part of the EDWP CCSP Waiver Program?
Yes, you can! Our intake team will help get your
initial screening done to see if you meet the preliminary
standards. Once complete, your referral will be sent to the
Area Agencies on Aging (AAA), where they will review your
financial eligibility for Medicaid. Once that is complete, you
will be scheduled for an assessment with one of our
nurses.
How much do CCSP services cost?
CCSP services could be free based on your
financial income and assets. If your financial income and
assets reach over a certain amount, you may be required
to pay for a portion of the services that you receive.
How long will I be eligible to receive CCSP Services?
You will be eligible to receive services through
this waiver program as long as you continue to meet the
financial requirements and nursing home level of care.
What makes you eligible for CCSP Services?
- Functional impairment caused by physical limitations.
- Approval by a physician for the need for an intermediate level of nursing home care (LOC) and the development of a care plan.
- Medicaid eligible or potentially eligible based on a financial screening.
- Medicaid eligible and admittance into the CCSP program.
- The client receives services in the home and community
rather than nursing homes. - Participation in one waiver program at a time.
Is there a waiting list for CCSP services?
Providing services through the CCSP EDWP
Waiver Program is finalized by how much funding is
available through the state legislature. Individuals in the
most need of services based on their health will have
priority over others.
