Community Care Services Program or CCSP is a program that the state of Georgia has established to help individuals who are elderly or functionally impaired to remain in their homes. This service is an excellent choice for those looking to stay out of the nursing home and receive services needed to help with their health-related needs. The CCSP program is run by the Department of Community Health administrators and can be utilized by anyone eligible.
What services are offered with CCSP?
CCSP services offer multiple programs that help keep people in their homes/communities and out of the nursing home. These services include:
– Health assessments conducted by a Nurse
– Case Management, a social worker who will be assigned to you, assist with your medical needs and document any health-related needs
– Personal Support Support Services
– Adult Day Health
– Alternate Living Services
– Emergency Response Services
– Structured Family Care
– Respite Services
– Home Delivered Services
– Companionship and social activities

Who is Eligible for CCSP Services?
– Physical impairments can adversely affect functionality, sometimes rendering a person unable to carry out the activities of daily living. It is essential to know that Alzheimer’s and dementia are physical conditions and are not eligible for these services.
– Care that is not being met
– Eligible for Nursing Home Placement
– Approval from a physician
– Medicaid eligible
– Health and safety needs can be met by using this program
– Is not on any other Medicaid waiver program
– Medicare services, such as hospice, do not meet these services’ exact needs
– Home Delivered meals are not the only service that is needed
– The home is free of illegal behavior and threats toward anyone who may enter the home
